Thank you for your interest in the Vermont Oxford Network. The Vermont Oxford Network is a non-profit voluntary collaboration of over 800 Neonatal Intensive Care Units from around the world. Please read the overview of the Vermont Oxford Network and feel free to look at our member list.

The efforts of the Vermont Oxford Network include a very low birth weight database of all infants admitted to participating centers weighing 401 to 1500 grams, an expanded database that includes all infants over 401 grams of birth weight admitted to the NICU, randomized controlled clinical trials and quality improvement projects. The Vermont Oxford Manual of Operations gives detailed instructions for data submission, data definitions and provides sample data forms in the index.

Membership entitles members to:

• Expanded Database Quality Management Report (sample report with data removed)
or
• Very Low Birth Weight Database Quality Management Report (sample report with data removed)
• Newsletters
• Participation in Clinical Trials
• Quality Improvement Projects
• Attend the Annual Network Meeting and Quality Congress.

Membership in the Network for the Very Low Birth Weight Database (401 to 1500 grams) costs $4000 for 2010 and 2011. The annual fee for the Expanded Database (all infants over 401 grams) membership is based on hospital size (AHA Data). Small hospitals (less than 250 staffed beds) are $4300 per year, medium (250-500 beds) are $5350 per year and large (500+ beds) or Children's hospitals are $6400 per year.

If you decide to join the Network, please fill out the membership application form and submit it by e-mail. This is an interactive PDF and will be submitted by email to our membership team. If you cannot submit this form by email, please complete it, print and fax it to Lynn Stillman at 802 865 9613. Please indicate on the application form your choice of database option since this will determine the invoice amount. Once we receive this application form, an invoice will be e-mailed to the financial contact that you designated on this form.

Upon receipt of your payment and the signed legal membership agreement, your membership will be activated. (please print out this agreement and sign it.)
